FIRST TIME WITH US?
If this is the first time booking with Exige, you will be prompted to complete our Intake Form once you’ve selected your desired:
- SERVICE,
- THERAPIST,
- DATE & TIME.
So please take the time to fill it out accurately (it must be filled out prior to your treatment). The document is used to collect your contact information, insurance provider, and health history.
It is an important form that will be used to provide you with receipts for your employer’s insurance provider.
We strongly encourage you to complete the form prior to your appointment so as to not interfere with your treatment time.
Should you choose to fill out the form upon your arrival, we will provide you with a tablet. Please give yourself plenty of time and arrive at least 20 minutes early.
We will be more than happy to answer all of your questions BEFORE your appointment, but you must show up early.
Understand that arriving any later will likely result in you cutting into your treatment time.
